Please copy and paste onto a text document the following questions. Answer each question. Then, paste the questions and answers into an email and send them to us. If you have not received an acknowledgement from us within 24 hours, please recheck you have our correct email address and re-send the information.

Note: questions marked with an asterisk (*) are required information.
The remaining questions will help us design a page that reflects you and your personality. Please answer them as best you can.

BASIC REQUIRED INFORMATION (WE CAN'T DO YOUR SITE WITHOUT THIS INFO :)

*1. What is the full name of the site coordinator? (Site coordinator is the person ordering and paying for the site.)

*2. What kind of site do you wish to display? (Choose one of Resume/Work Related, Hobby, Guild/Association)

*3. What name do you wish used as part of your site name? (Example: bettyscrafts, james-smith, circleofpeace, steelwarriors - this would then give a name such as www.bettyscrafts.twobells.com, www.james-smith.twobells.com, www.circleofpeace.twobells.com, or www.steelwarriors.twobells.com)

*4. What do you want your site to be called? (Example: Betty's Crafts Page, James Smith Curriculum Vitae, Circle of Peace Meditation Group, Steel Warriors EQ Guild)

*5. What is the City, State/Province, Zip/Postal Code of the site coordinator? (Note: This information is required for web site hosting partner)

*6. What is birthdate of the site coordinator - month, day, year? (Note: This information is required for web site hosting partner and web-based email provider if you wish an email account set up.)

*7. What is the site coordinator's correct email address? Please also provide an alternate email address if you have one.

*8. Do you have specific colors you want the site done in? If not, what are your three favorite colors?

*9. Which of the following styles would you like your website designed in? (Pick at least one - you may pick more if they don't contradict: bold, fantasy, modern, old-fashioned, Americana (Canadiana, etc.), futuristic, laid-back, casual, plain and simple, classical, retro, elegant, happy, dark, mysterious, comic, other. If you picked other, please describe.)

*10. If for a hobby or guild/association site, please describe in detail what the hobby or guild/association is all about.

*10. If for a resume/work-related site, please describe your skills, accomplishments and goals. This will be for the main page. You will also need to send the resume information.

*11. If for a hobby site, please advise which four pages you would like to include in your site (example - main page, display page, news and links page, message board page - or you may substitute any other pages you wish for those listed - up to a total of four pages).

*11. If for a resume/work related site, please advise if you wish a main page (as per the example), the resume itself (one or two pages), and references or other backup and support. You are entitled to four pages altogether. (Note: You will need to provide all the information. We will set it up to look professional, unless you have a definite display set-up in mind.)

*11. If for a guild/association site, please advise which four pages or combination thereof you want included. (main page, a roster, a charter (or rules and regulations) page, a guild display page, a forum, a chat room, a news page, a how to join page, and so on. If the volume of information is small, you may even combine two pages, such as a main page which includes news and current events - a charter page which includes how to join the club or guild.)

*12. Is there anything you don't want to see on your site?

*13. When do you need the website completed? (Please be fair - if you really need it in a rush, like for a birthday present or to please a friend, say so. If you would just like to have it as soon as possible but no particular rush, say so :)

THE FOLLOWING QUESTIONS ARE OPTIONAL (JUST REMEMBER THE MORE YOU TELL US, THE BETTER JOB WE CAN DO FOR YOU :)

14. Do you wish us to set up a web-based email service for you? If so, what user name do you want to use (please provide a few alternatives, as often the email providers will reject many names). Do you have any email provider preference?

15. Go to the Gallery. Let us know which backgrounds or logo styles do something for you.

16. Will you be sending photos or images? If so, by email or snail mail? (Remember, we will scan up to 5 photos for you - after that, it is $1 per photo, plus return postage, to be added to your bill. Or you can email them for free postage :)

17. Remember all specialized information must be provided by you, such as information on your hobby, work-related interests, or guild/association.

18. Anything else you'd like to say about your new website?

If you have any questions, please go to the FAQs re Display Sites page. Otherwise, if you have not already gone to the Order page and paid the deposit, please do so. Once we have your deposit, we will start work on your site. (Please bear with us. We may email you a few times to clarify things as we work on your site.)
